Comprehensive Community Health Centers, which opened its fifth location in Sunland in April, held an open house and ribbon cutting on Saturday, Sept. 16.
CCHC serves low-income, underserved, significantly immigrant, Latino, Armenian, and Asian communities of North Hollywood, Glendale and northeast LA with substantial numbers of patients from northwest and central San Fernando Valley, downtown Los Angeles, and the Hollywood area. Current programs and services offered include primary health care, family planning, prenatal and perinatal services, well-child services, behavioral health care, preventive services, diagnostic laboratory, enabling services, preventive and restorative dentistry, optometry, health education, community outreach, benefits counseling and enrollment services, and case management. CCHC has designed and developed the Sunland clinic to serve as a model Patient-Centered
Medical Home (PCMH) site. PCMH is a model of care delivery that emphasizes the use of systematic, patient-centered, culturally sensitive and coordinated care processes. In Sunland, the group will incorporate the PCMH processes, related professional development, patient engagement strategies and coordinated care using a team-based approach supported by cutting-edge technology platforms.
